Online FCN Theory Course Application
 


Registration for the FCN Theory Course (formerly FCN Part 1) is ongoing throughout the year.
                
Prior to completing this application, please carefully read all information available on the FCN Theory and Clinical Education Providers' pages of the Foot Canada Training website, including the Online FCN Theory Course Outline and Onsite FCN Clinical Course Outline. By submitting this application you are confirming that you have read this information.

If you selected discount criteria #4 (experienced Foot Care Nurse), please note that the following supporting documents are required.

Supporting Documents

  • Professional Reference - If wanting to be considered for this discount on the basis of being an experienced Foot Care Nurse, applicants are required to have practiced as a nurse for the last 2-5 years AND have a minimum of 750 nursing foot care practice hours within this timeframe. Applicants must have an employer or foot care health professional complete the Professional Reference form to confirm that applicant has met this experience prerequisite.

    The applicant may have more than one professional reference complete this form if each reference can only validate a portion of the total years of nursing experience or total hours of nursing foot care required. For example, if 300 hours are completed with one employer and 450 hours are completed with another, both employers will need to complete this form for the hours they are able to confirm. The total hours of both references would then be used to validate the applicant has met the prerequisite.
     
  • CAFCN Membership - Applicants must also email an electronic copy of their valid CAFCN membership card and/or membership purchase receipt showing current registration to footcarenurseprogram@gmail.com on the same day they submit their FCN Course Application (Part 1). The email subject line must contain the last and first name of the applicant and be formulated as follows: LAST, FIRST - CAFCN Membership.

1. The FCN Theory course uses a discussion board as well as group emails to communicate with the class. If accepted into the FCN Theory course I consent to my email address being visible to my classmates.

2. In submitting this application I am aware I am applying for the Online Foot Care Nurse Theory course (FCN Theory). FCN Theory offers a theoretical foundation to nurses preparing to take the Onsite Foot Care Nurse Clinical course (FCN Clinical). FCN Clinical provides in-person clinical skills development and mentorship. Together, both courses are designed to prepare participants to function as a Foot Care Nurse within a health care team. 

The curriculum for FCN Clinical was developed by Foot Canada Training and is administered by Foot Care Nurse Educators, colleges and universities who have a license to access and use Foot Canada Training’s curriculum. Education providers teach and administer FCN Clinical independently from Foot Canada Training. Each education provider has access to the same standardized peer-reviewed education guidelines and suggested Onsite FCN Clinical Course Outline. Please visit the Clinical Education Providers' page of the Foot Canada Training website for a list of FCN Clinical locations. 

Registration to FCN Theory does not guarantee a space for FCN Clinical at your location of choice for the upcoming session. We strongly encourage all students to contact their FCN Clinical location of choice and inquire about available courses and the registration process prior finalizing registration to FCN Theory.
